The Covid-19 pandemic caused application processing delays, which is why the USCIS decided to temporarily extend the validity period for medical examination, officially known as the Report of Medical Examination and Vaccination Record, Form I-693. Previously, the examination results were valid for two years, but now the validity period will last for four years, effective August 12.

The signature cannot be older than 60 days before you filed Form I-485. To save time, it is best to file both forms simultaneously.
